Reservation/Lock Documents
The Lender will print the OHFA Loan Confirmation and the OHFA Lender's Review Certification upon reservation/lock.

Certification & Approval Documents
The Lender submits the following documents to OHFA at [email protected] for certification:

• OHA Loan Confirmation
• Executed OHFA Lender's Review Certification
• Loan Transmittal/1008
• URLA (Uniform Residential Loan Application)
• VOR or Credit Report (three years reporting required for Non-Targeted residence/mortgagors)
• Loan Estimate
• OHFA Partial Exemption Disclosure - Second Lien
• Purchase Contract

OHFA provides these documents to Lender upon Loan Approval:

• OHFA Approval/Award Letter
• OHFA Second Lien DPA Letter
• OHFA MRB Disclosure
• OHFA Mortgagor Closing Certification

Closing Documents
The lender completes the following documents prior to Closing for mortgagor(s) to sign:

• Mortgagor Affidavit and Certification Part 5
• Tax Exempt Financing Rider
• Notice Potential Recapture Tax - Part II
• OHFA Partial Exemption Disclosure - Second Lien
• OHFA Subordinate Mortgage
• OHFA Promissory Note
• OHFA Mortgagor Closing Certification (sent to the lender at approval)

The Lender will provide the following documents to OHFA within 5 business days after loan closing:

• Copy of executed Closing Disclosure
• Copy of executed OHFA Second Lien DPA Letter
• Copy of executed OHFA Mortgagor Closing Certification
• Copy of executed OHFA Partial Exemption Disclosure
• Copy of executed OHFA Subordinate Mortgage
• Copy of executed OHFA Promissory Note
